We file insurance as a courtesy to our patients. The card must be present at the time of service, along with a picture ID. Please be advised that regardless of insurance the financial responsibility for services rendered is the patient’s.

We file insurance claims

We file insurance claims with the card presented at the time of service, and then bill the patient for the balance. If we are non-participating or services are not covered, we will bill the patient, and we do offer a self-pay discount. You will not automatically be turned away, as providing treatment is our focus – not insurance. If your decision on whether or not to have services will depend upon provider network participation, you should check with your insurance before receiving services.

We are contracted

Northwest Community Healthcare has agreements to provide healthcare services to participants of the managed care plans listed below . Physician participation in insurance plans may vary and if your plan requires a referral or PCP clearance, please call your payer to check.

We file insurance claims as a courtesy, but the patient is ultimately responsible for all charges. We do not turn away patients if we are not in-network, as we have many patients who choose to self-pay. We will do our best to help, but please remember that your insurance policy is a contract between you and the insurance company – we are not a party to that contract. If a patient seeks services, she will be billed the remainder of the balance after the insurance claim is processed.

Medicare – regular Medicare only – we are not contracted in any Medicare replacement plans. If your specific plan allows you to seek out services at any Medicare provider, they may still allow for services here. Please call your plan if you have any questions or concerns to check beforehand.
